Welcome to this year’s Lou Malnati’s Frozen Pizza Fundraiser!

    Contact a club member to purchase pizza coupons.
For Players/Parents:
    Please read the details below carefully to understand how the sale will be conducted.

1. This year’s fundraiser will run from Monday, September 13th until Sunday, September 26th. All orders will be required to be submitted no later than Tuesday, September 28th for processing. Completed order forms can be emailed to Kim Dahlin @ basketlover70@sbcglobal.net.

2. This order form is required to be submitted along with a check for the grand total of their order made out to Saint Viator Lions Hockey Club. Please, do not submit cash! Payment can also be made by Venmo to @Leah- Heneghan in lieu of a check.

3. Orders will take two (2) weeks for processing by Lou’s fundraising department and are tentatively scheduled to arrive the week of October 13th for delivery. Orders will be delivered to the team managers for distribution to players.

4. Each coupon is good for one 9” frozen DEEP DISH pizza (cheese, sausage, pepperoni, or spinach ONLY) to be picked up by customers at any Lou Malnati’s location in Arizona, Illinois, Indiana and Wisconsin. The coupon is good for 6 month; the expiration date will be at the bottom of the coupon. At this time, a gluten free option is not available with the coupons.

5. The discounted FROZEN deep dish pizza coupons will be purchased from Lou Malnati’s at the price of $8 per coupon. Players will sell the coupons for $13, the $5 profit from each coupon sold will benefit the Saint Viator Lions Hockey Club.

6. Each player should have the payment made out to them when collecting funds. At the end of the sale period, a single payment will be submitted.

7. Vouchers may be used for a frozen heart shape pizza during the holiday season, upon availability, and depending what is in stock at the location where the coupon is being presented.

8. These vouchers are not valid for shipping frozen deep dish pizzas. Coupons must be brought in the store to be picked up.

9. Certificates cannot be used towards any other food items
